A Clinical Study on the 9 Cases of Laryngeal Granuloma Following Endotracheal Intubation


Abstract
Laryngeal granuloma following endotracheal anesthesia was reported as traumatic granuloma of the, larynx by Clausen in 1932.
Thereafter Harrison reported that 4% of the intubated cases developed larynx and upper respiratory tract injuries and 1% of the above mentioned injured cases later developed laryngeal granuloma with symptoms including voice change and even dyspnea in severe cases.
The endotracheal intubation has been widely used for general anesthesia or patency of the air -way and the reported cases of the laryngeal granuloma as its complication have been increased in number worldwidely.
For a year March 1975 to February 1976, the authors have experienced the nine cases of laryngeal granuloma in our department and following results were obtained.
1) The age distribution showed mostly in the 21 to 40 years old group (8 cases), and the ratio of male to female was 1 : 8, predominant in female.
2) In all cases the site of granuloma was inner surface of vocal process on the arytenoid cartilage, and unilateral cases were 6, bilateral 3, and in unilateral cases all in the right side.
3) In all cases foreign body sensation on throat and hoarseness were complained and ¢¥the latter was more prominent.
4) Histopathological study revealed granulation tissue composed of proliferation of the newly formed small capillaries and fibroblast with marked inflammatory infiltrate in all cases.
5) In treatment, granuloma was removed under direct laryngoscopy in all cases and the recurrent cases were not seen.
